- If late registered, submit 2 original supporting documents indicating full name, date and place of birth and citizenship prior to date of late registration.
For changing of status from single to married: CHANGE OF SURNAME
- NSO Certified Marriage Contract
- Certificate of Attendance from CFO (Commission of Filipino Overseas) if married to a foreigner (color green).
For Minors (below 18 years old)
- Personal appearance of both minor and mother.
- Passport of person traveling with the minor, original and photocopy.
If both parents are abroad:
- Affidavit of support and consent indicating the name of the traveling companion and the relationship to the minor (must be authenticated by the nearest Philippine Embassy or Consulate General)
- Special Power of Attorney designating the representative by name and authorizing him/her to apply for a passport on behalf of the minor in their stead (must be authenticated by the nearest Philippine Embassy or Consulate General).
- Certified True Copy of parents' passport (must be authenticated by the nearest Philippine Embassy or Consulate General)
- Original and photocopy of the passport of person traveling with the minor.
- Original valid ID w/ signature or passport of the authorized representative.
- PRC / IBP ID
- Government Service Record
- School Form 137 or Transcript of Records
- Other documents that show full name and birth details of applicant and/or citizenship
- Baptismal Certificate
- Personal Appearance is required.
- Original Brown Passport
- Three (3) colored photos*
- Royal blue background and No shadow
- Applicant should be in decent attire with collar & sleeves.
- Photo must have been taken within the last six (6) months.
- Photo size: 4.5 cm x 3.5 cm.
*DFA has the right to reject photos that do not comply with the above requirements or meet standards
- NSO Certified Birth Certificate
- Personal appearance is required.
- Notarized Affidavit of Loss.
- Police Report – if lost passport is still valid
- Submit same documents required for first time passport applicants.
